Dr Fortune Siphiwe Hlatshwayo, trading as Dr FS Hlatshwayo Medical Clinic ("the Practice", "we", "us", "our"), respects your privacy and is committed to protecting your personal information in accordance with the Protection of Personal Information Act 4 of 2013 ("POPIA"). This Privacy Policy explains what personal information we collect through this website (https://www.drshlatshwayo.co.za), why we collect it, and how it is used, stored and protected.
Through our Booking form and Patient Information form, we may collect: your full name, date of birth, ID/passport number, contact details (phone, email, address), medical aid details, emergency contact details, and limited medical history information (such as known allergies, chronic conditions and current medication) that you choose to share with us ahead of your visit.

We do not use your information for marketing purposes without your separate consent, and we do not sell or rent your personal information to third parties.
We process your personal information on the basis of your consent (given via the checkbox on our online forms), and where necessary to provide you with medical treatment or to comply with a legal obligation.
Form submissions are transmitted securely and delivered directly to the Practice's administrative email, and an automatic confirmation email is sent back to the email address you provide, acknowledging receipt of your submission. We take reasonable technical and organisational measures to protect your personal information against loss, unauthorised access, alteration or disclosure. Only authorised Practice staff have access to your information.
We may share your information with your medical aid scheme (for claims purposes), referring or receiving healthcare providers involved in your care, and service providers who assist us in operating the Practice (such as our website host), under appropriate confidentiality obligations. We do not share your information with any other third party without your consent, unless required by law.
We retain patient records for as long as required by applicable healthcare record-keeping regulations and professional guidance in South Africa.
Under POPIA, you have the right to access the personal information we hold about you, request that it be corrected or updated, object to certain processing, and lodge a complaint with the Information Regulator of South Africa if you believe your information has been mishandled.
